Application Instructions

IMPORTANT: Before you apply, review General Eligibility Requirements and Scholarship Descriptions to ensure you meet special eligibility requirements for the scholarship(s) for which you are applying.

Information entered is automatically saved locally to your computer. You must continue your application on the same computer in order to use the saved information. It is recommended to complete your essays and gather your necessary verification documents prior to beginning the online application. Please review the Checklist for the required documents that you will need to complete this application. 

The CAPED Scholarship Application is a four-part application:

  • Part 1: Personal Information
  • Part 2: School Information
  • Part 3: Scholarships & Essays
  • Part 4: Verification & Recommendation Documents

All parts of the application are required for consideration. You can use the checklist provided in our forms to assist you through the process.

STEP-BY-STEP GUIDE TO APPLY

PART 1: PERSONAL INFORMATION

Complete all fields. 

PART 2: SCHOOL INFORMATION

Complete all fields. Current unofficial transcripts are required. 

Save your transcripts with the following naming format: Last Name, First Name_Transcripts.  For example: Smith, John_Transcripts

Upload your transcripts as a .doc or .pdf attachment in the appropriate field.  

PART 3: SCHOLARSHIPS & ESSAYS

Scholarship Selection

Choose and click up to 3 scholarships to apply. Any special requirements for scholarships will be listed next to the scholarship name.

Note: selected applicants will only receive 1 scholarship award

Essays

Complete the essay question required by the specific scholarship(s) for which you are applying. You have the option to write an essay or record a video essay. Essay prompts are listed on the Scholarship Application

Note: Each scholarship has a unique essay prompt. This means you may have to answer up to 3 different essay prompts if you apply for multiple scholarships.

If writing your essay:

  • Place your full name and the specific scholarship(s) for which you are applying at the top of each of your essays. For example: John Smith, Susan Bunch Memorial Scholarship
  • Save your essay with the following naming format: Last Name, First Name_Scholarship Name. For example: Smith, John_Susan Bunch Memorial Scholarship Essay
  • Upload essay as a .doc or .pdf attachment in the appropriate field. 

If recording a video essay:

PART 4: VERIFICATION & RECOMMENDATION DOCUMENTS

LETTER OF RECOMMENDATION

One letter of recommendation from a college/university instructor/professor (not a disability specialist or counselor) is required. This letter must describe your academic achievements and your ability to meet scholastic responsibilities. This letter must be signed by the person writing it (electronic signatures will be accepted). Note: We cannot receive any letters by email, they must be attached to your application packet. 

Save your letter of recommendation (LOR) with the following naming format: Last Name, First Name_LOR. For example: : Smith, John_LOR

Upload letter of recommendation as a .doc or .pdf attachment in the appropriate field. 

DISABILITY/ACCESSIBILITY CERTIFICATION FORM

Download Disability Certification Form. Ask your college/university Disability Office to complete and sign form. Note: We cannot accept any other form of disability verification. 

Save your Disability Certification Form with the following naming format: Last Name, First Name_Disability Certification. For example: Smith, John_Disability Certification

Upload completed Disability Certification form in the appropriate field.
